Location: Sunshine Hospital Employment Type: Fixed Term, Part-Time (until 1 November 2026) Salary: Community Health Nurse classification About the role: Western Health at Home Connect is seeking an passionate nurse who wants to provide high level care and support to our community as an Overnight Community Liaison Nurse.
This is a part-time (0.5 FTE), fixed term role until the 1st of November 2026.
As a Community Liaison Nurse you will work from the Western Health at Home Hub based at Sunshine Hospital and receive calls from residential aged care facilities, patients, family members, carers, and a variety of other community stakeholders and provide high level evidence based triaging services, advice to consumers, arranging community follow up at home from a Western Health at Home clinician.
This role also involves offering support to the community nursing teams by helping to manage and allocate workloads and schedules for the following day to the Western Health at Home teams.
Key responsibilities: Triage incoming calls from community stakeholders and Western Health at Home patients.
Provide evidence based advice to community stakeholders to help manage their condition in the community.
Escalate care appropriately.
Support Western Health at Home teams with planning for the following day (schedules, workloads).
Liaise with a variety of stakeholders to ensure patients are well supported.
About you: Be a Registered Nurse, currently registered with AHPRA.
Experience working in senior nursing roles with a demonstated ability to deliver high quality patient centred care.
Possess the ability to work independently and without supervision.
Highly organised and have impeccable time management skills.
High level verbal and written communication skills.
Technologically capable with experience with proficient ability in using Microsoft Excel and Word.
Commitment to developing and improving personal education and skills.
