At Directions Health Services, we believe in the power of connection, compassion and non-judgemental care. For nearly 50 years, we've been an innovative, not-for-profit leader providing holistic alcohol and other drug (AOD), mental health, and primary health services across the ACT and NSW.

The T25 Program brings together multidisciplinary expertise to support young people with complex mental health and co-occurring needs through integrated, community-based care. We're now seeking an experienced and values-driven Mental Health and Co-occurring Needs Clinician to join this dedicated team.

About the Role

As a Mental Health and Co-occurring Needs Clinician, you'll work within a multidisciplinary team and in partnership with Woden Community Service to deliver coordinated, trauma-informed and culturally safe care to young people and their families. Your work will include assessment, therapeutic intervention, group facilitation, outreach and case management all guided by a recovery-oriented, person-centred model of care.

This role is ideal for a clinician who values collaboration, flexibility and meaningful engagement, and who is passionate about improving outcomes for young people experiencing mental health concerns alongside other co-occurring challenges.

This is a full-time position on a fixed term contract with extensions available.

What You'll Be Doing:

* Provide integrated, holistic care within a primary health setting to meet the needs of young people and their families
* Deliver trauma-informed, culturally safe, person-centred and strengths-based interventions
* Undertake mental health assessments, counselling, case management, group facilitation and support services
* Work collaboratively within the T25 multidisciplinary team to coordinate care and contribute to shared treatment planning
* Apply a "no wrong door" approach to ensure young people can access the support they need
* Identify goals with clients and develop individual treatment and support plans
* Provide assertive outreach and mobile services to maximise engagement
* Support access to primary health care, mental health interventions and residential treatment as required
* Build and maintain strong relationships with health, community, justice and emergency service partners
* Work collaboratively with families and carers, with the young person's consent
* Ensure all practice aligns with the National Principles for Child Safe Organisations and Directions policies
* Maintain accurate documentation, outcomes measurement and data collection
* Actively contribute to quality improvement, safety initiatives, supervision and team meetings

About You

You're a compassionate, skilled clinician who thrives in multidisciplinary environments and is committed to delivering safe, inclusive and effective care to young people with complex needs.

You bring:

* A qualification in Counselling, Social Work, Psychology, Nursing or a related field
* Cert IV in Alcohol and Other Drug Work and/or Mental Health, or willingness to obtain
* Extensive experience working in health or community services with young people experiencing complex and co-occurring needs
* Strong understanding of mental health, AOD, physical health and other co-morbidities
* Knowledge of evidence-based mental health and AOD interventions
* A trauma-informed, recovery-oriented and person-centred approach
* Commitment to harm minimisation and culturally safe practice
* Understanding of the social determinants of health
* Excellent interpersonal skills and confidence supporting young people in distress
* Ability to work collaboratively within a multidisciplinary team
* Experience building partnerships with external services and stakeholders
* Cultural competence, including understanding issues impacting Aboriginal and Torres Strait Islander communities
* Strong organisational skills and confidence using electronic client management systems
* Willingness to travel across the ACT and NSW

Requirements

* Current Driver's Licence
* First Aid Certificate (or willingness to obtain)
* AFP National Police Check
* ACT Working with Vulnerable People Registration
* NSW Working with Children Check
* Australian Citizenship or suitable rights to work in Australia
