Job Description
A support coordinator plays a vital role in empowering individuals with disabilities to achieve independence. Their key responsibilities include:
* Connecting service providers with customers and linking them to relevant services
* Developing and refining customer goals through collaborative meetings
* Engaging with customers' informal supports, such as family and carers
* Building knowledge of local services and organisations that customers can utilise
* Establishing relationships with government agencies and local community groups
* Providing crisis support to customers
* Assessing and managing risk in line with organisational policies and procedures
Required Skills and Qualifications
The ideal candidate will possess:
* A sound understanding of the NDIS framework and operational guidelines
* High emotional intelligence
* Experience in the Community Services field with strong community connections and networks across NDIS providers, mainstream supports, and the community sector
* Exceptional time management and scheduling skills
* Ability to work across multiple priorities across caseloads
* Tertiary qualifications or substantial experience working within the NDIS framework
* Well-developed communication skills with the capacity to engage at all levels
* Ability to work well in a changing environment
* Self-starter with high degree of initiative and problem-solving skills across multiple sectors
* Experience in working in a team environment that is open and transparent with a person-centered focus
