We value professionalism, fun, flexibility, and a team culture that treats employees like family.About the role:Support coordination helps NDIS participants to make the best use of supports in their NDIS plan. It is a capacity-building support that assists participants to understand and utilize their NDIS plan to pursue their goals, connect with providers and community services, and build confidence and skills in support coordination.Psychosocial recovery coaching supports NDIS participants with psychosocial disabilities to increase independence, social and economic participation, and to manage daily challenges. Coaches perform all support coordination tasks but with a focus on psychosocial recovery, requiring in-depth knowledge of mental health or lived experience with psychosocial conditions.Requirements of the role:Responsibilities include:Onboarding participants for support coordination/psychosocial recovery coaching.Meeting participants/supports in homes or community settings to implement NDIS plans.Educating participants/supports about NDIS, community, and mainstream services.Connecting participants with providers and services.Building confidence and skills in support coordination.Conducting person-centred planning, risk assessments, and emergency planning.Procuring assistive technology and consumables.Prospecting accommodation options.Setting up service provider accounts and completing referrals.Drafting and forecasting NDIS budgets.Monitoring progress and budget tracking.Investigating anomalies and resolving issues with providers.Collaborating with stakeholders and reporting progress to NDIS.Managing complaints and assisting with plan reviews.For Psychosocial Recovery Coaches: All above responsibilities plus coaching to build strengths, resilience, and decision-making skills.Education / Qualification Requirements:Relevant qualifications in Disability, Mental Health, or Allied Health or equivalent experience.First Aid and CPR certifications.Highly desirable: Mental Health First Aid.Skills / Knowledge / Experiences:Substantial experience as a Support Coordinator, Psychosocial Recovery Coach, or similar.Familiarity with psychosocial disabilities.Completion of NDIS mandatory training modules.Proficiency in Microsoft Office 365 applications.Sound IT and office equipment skills.Licences / Clearances / Requirements:Own laptop, printer, scanner, and ergonomic home office setup.Reliable smartphone and internet.NDIS worker screening clearance, Working with Children Card, Police Check.Queensland driver’s license and own vehicle with insurance.What is on offer:Casual employment with above-award pay, potential for permanent role.Hours grow with caseload.Benefits include superannuation, WorkCover, QLeave, subscriptions, and reimbursements.Work from home, social events, mentoring, and training.Next steps:Download the full position description from our website’s 'Contact Us' page: www.chrysaliscollective.au.To apply, send your resume and a 2-3 page cover letter to hello@chrysaliscollective.au.
