About the role
The specialist clinician, working as part of a multidisciplinary team, is responsible for coordinating the care of clients seeking services within their specialty area and providing expert and quality clinical services in collaboration with consumers, carers, and other care providers, using an integrated approach. This position will undertake the role of Assessor for the Single Aged Care Assessment Service and provide client-end assessment.
The Aged Care Services Division delivers a range of clinical and non-clinical services across the Western NSW Local Health District to support older people and their families, enabling them to remain supported in their homes.
The Aged Care teams and programs are committed to advocacy, restorative care, and reablement to restore wellness and independence. The position offers cooperation and support within a strong leadership team and involves navigating challenges in a changing aged care environment responding to a growing patient population and health reforms.
