For how to apply and mandatory requirements, please refer to the attached Role Description.Your opportunityTo provide high-quality support services to frail, older people to maximize their independence at home and in the community, supported by a motivated, consumer-focused Home Care Coordination Team, liaising closely with Community Allied Health Professionals.About The TeamMackay District Home Care Service is funded by the Commonwealth Home Support Program (CHSP) to provide accessible, flexible, and goal-oriented basic support services. We are also an Approved Aged Care Provider, offering Client Directed Home Care packages to eligible consumers.Our service provides basic support, mainly Domestic Assistance and Social Support, to approximately 1000 consumers within the Mackay district.Potential CHSP consumers are registered and screened via “My Aged Care,” and if eligible, referred for assessment by a Regional Assessment Service (RAS). This assessment helps frail aged people, people with disabilities, and their carers to live independently in the community.The Mackay District Home Care office is located at the Carlyle Community Health Centre, 40 Carlyle Street, Mackay.About Our Hospital And Health ServiceMackay Hospital and Health Service comprises 6 hospitals, 2 multi-purpose health services, and 4 community health centres, employing over 3,700 staff.
