About the job
As a dedicated Nurse Concierge within one of Five Good Friends' strategic village partnerships, you are the health and wellbeing touchpoint for residents — providing advice, support and education to help them age well, stay independent, and remain connected to their community.
The role draws on your clinical expertise and relationship-building skills to deliver Five Good Friends' "Move.Nourish.Connect." wellness program and to coordinate care planning in collaboration with residents, village teams and external providers. You will act as a trusted advocate and ambassador, supporting residents through the full ageing journey — from health education and social connection to navigating aged care systems.
The day-to-day ranges from:
* Deliver proactive wellbeing support to help residents stay healthy, independent, and connected.
* Provide guidance on My Aged Care and other care pathways, including support with assessments and care planning.
* Collaborate with the Care Team to tailor in-home care solutions based on individual needs.
* Design and lead wellness activities aligned to residents' goals using the "Move.Nourish.Connect." framework.
* Build trusted relationships with residents, families, and healthcare providers to support smooth transitions and ongoing care.
* Support new and prospective residents by identifying care needs and easing their transition into the village.
* Maintain accurate records and contribute to resident wellbeing planning and village wellness reporting.
* Deliver on promises and always try to exceed partner expectations.
* Be curious and look for new ways to improve process, grow our brand locally and deliver value back to our partners.

Experience / Qualifications
* Tertiary qualification in a health-related field (e.g. Bachelor/Diploma of Nursing or Allied Health).
* Current AHPRA registration (as required).
* Previous experience in community or aged care settings.
* Knowledge of aged care systems and funding options, including My Aged Care.
* Experience with health education, event facilitation or program delivery within a wellness model is desirable.
* Familiarity working in retirement villages or supported living environments is desirable.
* Passion for supporting older Australians to live meaningful and independent lives.
* Ability to develop strong and trusting relationships with a wide range of stakeholders.
* Excellent communication and interpersonal skills.
* Well-organised, self-directed, and able to manage competing priorities.
* Technologically confident; able to use digital platforms for care planning and data entry.
* High level of integrity, confidentiality, and professionalism in all interactions.
* Current driver's license and National Police Check.
* You must have the right to work in Australia.
