Clinical Care Liaison

Make a meaningful difference in the lives of people living with progressive neurological conditions. We're seeking a Clinical Care Liaison—a proactive, compassionate Registered Nurse or Allied Health Professional with excellent communication skills and strong attention to detail—to join our dedicated Statewide PND Service team.

About SPNDS

The Service provides multidisciplinary assessment, education, and management for people with complex progressive neurological conditions including:

* Huntington's Disease
* Motor Neurone Disease
* Multiple Sclerosis
* Muscular Dystrophy
* Parkinson's Disease
* and other progressive neurological disorders

Our mission is to enhance quality of life for patients and their families/carers by delivering holistic, person‐centred care that supports physical health, psychosocial wellbeing, and disability‐related needs.

About The Role

As a Clinical Care Liaison, you will play a key role in supporting clients living with progressive neurological diseases by providing expert advice, care coordination, and ongoing management support. This role combines clinical expertise with relationship‐based, collaborative work across a multidisciplinary team.

Key Responsibilities

* Coordinate client care through phone triage, needs assessment, appointment scheduling, shared care planning, and liaison with interdisciplinary and multidisciplinary team members.
* Manage a clinical caseload and work closely with Neurologists to deliver specialised, high‐quality care.
* Provide telehealth consultations and secondary advice to community health providers and external partners to enhance care across the sector.

Internal: All Calvary Health Care Bethlehem staff

External: Community service providers, professional networks, and relevant peak bodies
