In this pivotal leadership role, you will:KeyResponsibilitiesOversee resident assessments and develop tailored care plansCoordinate clinical documentation and compliance reporting (AN-ACC)Lead and mentor a team of RNs, care staff, and volunteersCollaborate with external providers to ensure seamless care deliveryMonitor and improve clinical programs and quality indicatorsChampion a culture of safety, wellbeing, and continuous improvementAbout YouAs a Care Coordinator, you'll play a pivotal role in leading clinical care delivery for residents with complex needs. You'll oversee assessments, care planning, and compliance, while mentoring a team of Registered Nurses, care staff, and volunteers. Your leadership will ensure our residents receive holistic, person-centred care aligned with our Positive Wellbeing Model.This is a key leadership position reporting to the Service Manager, with direct responsibility for clinical staff and indirect influence across the broader care team.Requirements:AHPRA Registration as a Registered Nurse (RN) with a current Practicing CertificateMinimum 3 years' post-graduate experience in aged care or acute clinical settingsProven leadership and workforce management skillsStrong clinical assessment, care planning, and documentation capabilitiesIntermediate computer literacy (Word, Excel, Outlook, PowerPoint)A commitment to quality, safety, and continuous improvementCurrent National Police Check - or willingness to acquireCurrent First Aid & CPR certification - or willingness to acquireA Valid driver's licenceDesirable:Postgraduate qualifications in Gerontology, Dementia, or Aged CareExperience with mobile clinical documentation systemsAbout UsWe are one of Australia's largest and most diverse not-for-profit organisations, with a legacy of care and compassion spanning over 140 years.
