The newly created ACOP opportunity at Hall & Prior Health & Aged Care Group
Work three days at our 114 bed aged care facility in Lyneham
Report into a Senior Clinical Pharmacist and be part of a rich clinical environment with Nurses, Physiotherapists, Speech Pathologists and Dieticians
Deliver onsite clinical pharmacy services including medication reviews
Participate in family case conferences
Provide education and training to staff
Be involved in clinical governance including conducting audits and contributing to policy and procedures

About you
Bachelor in Pharmacy and current registration with the Pharmacy Board of Australia and AHPRA
Medication Management Review (MMR) Credential and a commitment to obtain an ACOP credential
Excellent communication skills, able to develop productive relationships and enjoy working collaboratively with a multidisciplinary team
Proactive, driven and passionate about making an impact in the aged care sector
NDIS Worker Screening Check and Influenza vaccination (or willing to obtain)
