Diabetes Educator Role Overview
This exciting opportunity invites a motivated Diabetes Nurse Educator to collaborate with our multidisciplinary team in providing patient-centered complex clinical nursing services.
* You will work primarily in an outpatient community health setting, delivering direct patient care and self-management education through face-to-face clinics and group sessions.
* Additionally, you will offer telehealth services and contribute to inpatient care initiatives.
As a key member of the team, you will engage in service planning and evaluation, policy formulation, education resource development, practice guidance, and quality improvement activities.
Notably, you will serve as a clinical leader and mentor, supporting colleagues and students in their professional growth.

The Chronic & Complex Care team operates within the broader primary and community health framework, collaborating closely with hospital and community teams to promote health and wellbeing.
