Purpose of the Position:
Abilifly Support Services is a registered National Disability Insurance Scheme (NDIS) provider with an office in Cairns. We specialise in providing in‑home support and community support/access to people with a disability, aged, children and young adults. As a Support Worker you will:
* Assist people to perform important daily activities.
* Facilitate daily personal hygiene and dressing tasks.
* Perform home‑based tasks such as food preparation and housework.
* Undertake tasks outside the home such as shopping.
* Ensure a varied programme of social activities.
* Provide emotional support and friendship.
* Support clients to participate in age‑appropriate activities and tasks including employment, in‑home support and community access.
* Support clients to access activities and outings in the community.
* Provide direct care assistance to each client in accordance with their service plan and as directed by the Manager.
* Work in accordance with the medication administration policy, work health and safety procedures, and client rights policies including privacy and confidentiality.
* Monitor the safety and well‑being of the clients and report any concerns to the Manager.
* Provide 1:1 or small group training and support in response to individual client needs.
* Ensure completion of appropriate documentation.
* Record relevant information on support provided in client notes and communication book as required.
* Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
* Work with clients, family/carers, relevant case managers and other professionals to support the client's individual goals.
* Assist in developing, implementing and evaluating training and behaviour programmes to attain identified individual goals.
* Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
* Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
* Some travel may be required to clients who are outside of the Cairns region.
Essential Criteria:
* Physically fit and active.
* The ability to develop a positive relationship with the participant.
* Excellent communication skills.
* The ability to work with children with disabilities.
* Able to work in a fast‑paced environment.
* Ability to acquire NDIS Worker Screening Check.
* Ability to acquire a Working With Children's Check.
* Covid‑19 Vaccination Certificate.
* First Aid Certificate is desirable.
* Cert III in Individual Support or Cert IV in Disability (or equivalent) is desirable.
