What You'll Be DoingLeadership & Case Management
* Oversee the delivery of high-quality care across clients, aides, and nurses
* Manage skilled nursing referral intake and ensure seamless coordination with families and providers
* Conduct assessments and supervise care planning within a person-centered model
* Provide hands-on clinical care as needed (wound care, IV/IG infusions, injections, etc.)
* Guide and mentor care staff with warmth, clarity, and high standards
Regulatory & Administrative Oversight
* Ensure compliance with Joint Commission standards, state regulations, and brand protocols
* Conduct chart reviews, incident reporting, and documentation audits
* Lead staff education, conduct supervisory visits, and complete orientation and skills training
* Participate in office on-call rotation and respond to urgent clinical issues(typically phone-based triage and support for clients and staff)

Job Responsibilities
Case Management and Patient Care:
* Oversee delivery of care to ensure quality across aides and nurses
* Coordinate care with other providers, clients and their families, and care staff
* Create and maintain plans of care within a person-centered framework
* Obtain physician signatures for all skilled plans of care
* Manage skilled nursing referral intake and follow-ups with referral sources, clients and families, other care providers, etc.
* Conduct initial admission assessments, re-assessments, and safety evaluations for skilled and non-skilled home care clients, when needed
* Provide patient care, when needed, as defined within the scope of the Nurse Practice Act - Ohio Revised Code (ORC), such as Infusion Therapies (e.g., IV/IG), wound care, injections, and more
Administrative:
* Conduct chart reviews weekly to ensure ongoing compliance with state regulations, Joint Commission (JCAHO) and brand standards, including but not limited to signed physician's orders, authorizations, and supervision over high-risk medications
* Manage incident reporting (i.e., reviewing and follow-up as needed to ensure positive resolution)
* Daily review of nurse's notes and other documentation to ensure all required notes are complete and turned in timely to avoid any delays in invoice/claims reimbursement
* Complete timely discharge of clients with a thorough discharge summary
* Assure proper maintenance of client clinical records, statistics, reports, and records for purposes of evaluation and reporting
* Understand and uphold existing agency policies and procedures
* Conduct caregiver and client introductions as needed
* Complete staff supervisory visits to ensure compliance
* Field staff education and ongoing training (both skilled and non-skilled), including but not limited to weekly orientation and competency demonstrations, and basic nursing skills and documentation
* Participate in office on-call rotation
* Ensure availability to manage critical clinical issues
* Attend and participate in office meetings as needed
Job Requirements
* Unrestricted and unencumbered license as a Registered Nurse in the State of Ohio
* Degree in nursing from a National League of Nursing accredited program
* Minimum three (3) years of documented clinical experience with at least one (1) year of case management experience
* Preferred: At least one (1) year experience in a home health agency
* Strong clinical skills. Self-directing with the ability to work with little direct supervision
* Impeccable communication and professional skills-ability to read, write, speak and understand English fluently
* Understand regulatory compliance with state and federal regulations
* Adhere to HIPAA and maintain client confidentiality
* Demonstrated ability to supervise and direct staff
* Possess exceptional nursing assessment skills
* Proven ability to build quality and implement improvements within the operation
* Comfortable with technology
* Excellent customer service skills and ability to deal tactfully with clients, staff, and community
* Ability to travel within the assigned territory daily including weekends, nights, and holidays
* Ability to pass drug, background, and criminal record checks
* Certified CPR and negative TB skin test or chest x-ray
* Up-to-date vaccinations including Influenza, COVID-19, Tetanus, Diphtheria, Pertussis, Rubella, Varicella, MMR, and Hepatitis B
* Licensed driver with reliable and insured automobile (proof of insurance required)
* Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client
* Must be able to lift up to 50 pounds
* Ability to travel as needed within Cuyahoga, Lorain, and Medina counties
