QIFVLS is a state-wide Community Legal Practice with office across 8 regions in Queensland. We are a not-for-profit organisation that provides free legal services, including court and tribunal representation, welfare support, advocacy and community education to Aboriginal and Torres Strait Islander victims of family violence and/or sexual assault. Currently we have a vacancy for a Case Management Officer (CMO) in our Case Management Services Unit for the Cairns Office.
Please note, that to perform this role, it is essential that the person who holds the position be an Aboriginal or Torres Strait Islander person and is therefore a genuine occupational requirement under section 25, 104 and 105 of the Anti-Discrimination Act 1991 (Qld).
About the role
Empower at-risk, isolated, and vulnerable individuals by utilizing a client-centered, strengths-based approach within a case management practice framework. Provide direct client services, including intake and assessment, support planning, and coordinating access (referrals) to a diverse range of community.
Responsibilities
Case Management
End-to-end case management including intake, administer client needs assessments, drafting Case Management Plans (CMP), service delivery plans, Case Management Plans (CMP) changes, regular reviews, and exit plans.
Appropriately assess and prepare individual (or family group) risk assessments, safety planning, and implement strategies to enable individuals to achieve their individual goals.
Engage and support clients to make informed decisions by implementing structured decision-making regarding their Case Management Plans (CMP) goals and available options to progress equitable solutions.
Promote quality and appropriate preventative and diversionary interventions to assist clients/families in achieving outcomes.
Coordinate and initiate culturally and physically safe, timely, effective, equitable and client-centered support, including documenting and actioning external social supports and referrals
Ensure high levels of privacy and confidentiality are maintained, by keeping consents and authorities up to date, aligning them with client's current goals and actions in the Case Management Plans (CMP).
Report any/all challenges, issues or complex cases with Case Management Team Leaders (CMTL) including open communication, debriefing sessions post incidents and high-risk situations
Records Management:
Accurately record client records in a timely manner, including daily file notes, case plans and reviews and data entry
Complete all necessary forms, checklists and Case Management Plans (CMP) aggregated data to generate summarized statistical reports. Maintain current, accurate, and relevant client records for reporting, evaluation, and information sharing, ensuring alignment with QFIVLS file management and reporting standards.
Effectively maintain and systemically update regional key stakeholder databases, registers, community profiles and client management system
Organize, manage, maintain and remain accountable for own work schedules, using tools and calendars, maintaining self-direction in planning and prioritizing tasks to remain autonomous in task completion
Compile and submit two case studies to Case Management Team Leader every three months/quarterly
Ethical Practices and Compliance
Foster critical thinking and ethical decision-making by encouraging reflective practices and adherence to case management ethical standards
Contribute to continuous improvements in compliance and risk management practices
The person we seek will be personable, caring and possess excellent organisation skills. A current open drivers licence is mandatory for this position, along with willingness to provide a national police clearance and eligibility to apply successfully for a Queensland Working with Children Check (Blue Card). Full training is provided for this position, including the opportunity to participate in nationally recognised formal qualifications through Case Management Standards Australia.

Hold relevant qualifications in Community Services Certificate III, Case Management, Counselling, Social Work or related field &/or possess qualifiable equivalent and extensive work experience in case management or similar role.
Holder of a current Open C Class Queensland driver's license.
Holder of a Working with Children Blue Card, or the ability to obtain one.
Willingness to undertake a National Police Clearance check.
