We are seeking enthusiastic and compassionate individuals to join our Support Worker (Community Based) team located South of the River.

No two days are the same in this dynamic role One day you could be helping a participant build their independent living skills and the next day you could be out and about providing recreational support and community access. This is a rewarding and varied role
What You'll Do
* Provide person-centred support to participants
* Assist participants in achieving their personal goals
* Promote community inclusion and social engagement
* Assist participants with community outings which may include recreational activities, appointments and religious activities
* Assist participants with their daily living skills and activities within the home

What You'll Bring To The Role
* Minimum 2 years of experience in the disability sector
* Passion for working with people in their homes and the community
* Ability to work autonomously and as part of a team
* Strong communication skills and the ability to build rapport with participants and families
* Ability to work across weekdays and weekends as per the required roster
* Ability to use your own vehicle travelling within the Perth metro area.
* Commitment to the NDIS Standards and the Safeguarding of children and adults at risk.
What You'll Need
* Australian Citizenship or Full and unrestricted working rights for Australia
* NDIS Worker Screening Check
* Current Provide First Aid Certificate
* Western Australian 'C' driver's licence and reliable motor vehicle with comprehensive car insurance
* Working with children and DCP Check (willing to obtain)
Availability
We require our workers to be able to do a mix of morning and afternoon shifts and weekday, weekend shifts.
Shifts will be located South of the River from Fremantle to Cloverdale to Piara Waters. They will be from 7am – 5.30pm Monday to Saturdays.
Selection Criteria
* Demonstrated experience supporting people with disabilities 1:1 to access their local community and achieve their goals. Please provide a summary of your experience and knowledge providing person-centred support for people living with a disability in the community setting.
* Flexibility and availability to work from 7am – 5.30pm Monday to Saturdays. Describe how you will be able to work within the required availability to support people who live south of the river.
* High level of accountability and responsibility. Describe your approach to teamwork and accountability in a support setting.
* Strong verbal and written communication skills. Provide evidence and examples from your previous work experience that supports your ability to communicate effectively in the workplace
* Ability to handle challenging situations calmly and effectively. Provide an example from your previous work experience where you successfully managed a crisis or difficult situation.
