**Our journey**
The **Victorian Heart Hospital** will be Australia's first dedicated heart hospital and is nearing its final stages of construction. It will include a full range of ambulatory and inpatient cardiac care, including cardiac theatres, cardiac catheterisation laboratories, emergency, intensive care and ambulatory services such as cardiac CT, echocardiography and specialist consultation. It will also feature a unique, patient centred model of care. The Victorian Heart Hospital will also play a key role in advancing research and teaching and will be home to the Victorian Heart Institute, a brand new, health and medical research organisation at Monash University.
**About Social Work**
The Social Work team across Monash Health is responsible for the delivery of a broad range of Social Work service for patient and their families across the lifespan in the inpatient and outpatient setting.
The Social Work Department is looking for an experienced Grade 2 social worker to join our growing team at Monash Health, Victorian Heart Hospital. **A Day in the Life**:

In this important role, you will have the opportunity to showcase your skills including:
- Working with people from a range of diverse backgrounds
- Complete complex psychosocial assessments,
- Development of patient centred care plans and complex discharges
You will be a member of a dynamic and supportive **Social Work and Allied Health** team working in a collaborative model of care across Monash Health. The position works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting.
The incumbent will be required to work in any clinical area at any or all campuses of Monash Health as and when required. Such a requirement may be a temporary arrangement, on an ad hoc basis, or a permanent change of location.
Participation in the Social Work on-call roster is required. Use of a personal vehicle with appropriate insurance for business purposes may be required.
**What You Bring**
Working closely with the Grade 3, Clinical Grade 4 and Social Work Managers you will bring the following demonstrated skills, experience and knowledge:
- The Grade 2 Social Worker will be an experienced and skilled clinician with consolidated psychosocial assessment and planning skills and able to use evidence to inform practice
- An understanding and working knowledge of the range of the service systems including Family Violence, Child Protection, My Aged Care, Transition Care Program, and the National Disability Insurance Scheme (NDIS), Mental Health and the recovery model is essential
- You will also provide supervision of clinical staff which may include Grade 1 Social workers, Allied Health Assistants and students, and contribute to service development. There is an expectation that effective, high quality care to patients across the health service will be provided
