About The Role:
* Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for clients in their own home through home care packages/NDIS.
* Work holistically with other allied health professionals to provide a multidisciplinary care plan.
* Educate clients and their families to optimize their ability to live healthy lives within the community.
* Liaise with service providers to ensure effective communication.
* Work autonomously to prioritize and complete tasks, maintain records and provide patient reports.
