The Specialist Palliative Care Service is based at Long Jetty Health Care Campus and provides outreach services across the Central Coast region. In this role, you will deliver care across a range of clinical settings, including acute hospital wards, patients' homes, and Residential Aged Care Facilities (RACFs), ensuring patients and families receive compassionate, high-quality support wherever care is required.


About The Opportunity

The Central Coast Local Health District (CCLHD) is seeking a dynamic and experienced Clinical Nurse Consultant Grade 2 to join the Specialist Palliative and End of Life Care Service based at Long Jetty Health Care Campus. This permanent full-time opportunity is ideally suited to a motivated clinician who thrives in a collaborative, community-based environment and is committed to delivering high-quality, person-centred care to patients and families during the final stages of life.

In this role, you will provide expert clinical consultancy and support to patients, carers, families, and multidisciplinary teams across a variety of community settings, including patients' homes and Residential Aged Care Facilities (RACFs). You will work closely with healthcare professionals to ensure complex palliative care needs are effectively managed, supporting patients with dignity and compassion through to end-of-life care and death.

Key responsibilities include delivering evidence-based clinical practice, leading quality improvement and risk management initiatives, contributing to clinical service planning, and providing education and support to multidisciplinary teams. You will also play an integral leadership role through mentoring, clinical supervision, and supporting the professional development of nursing staff.

The position primarily operates Monday to Friday, 8:30am to 5:00pm.

This is a varied and rewarding role that requires strong communication, clinical reasoning, and problem-solving skills, along with demonstrated experience in palliative or complex clinical care. Proficiency in Microsoft Word, Excel, Outlook, and electronic medical records (eMR) systems is essential. A comprehensive orientation program is provided to support new team members and ensure confidence across all aspects of the role.

About You

CCLHD is looking for you if you:

* Have a current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse (Division 1) with at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the position specialty and approved postgraduate nursing/midwifery qualifications relevant to the position specialty field or other such qualifications or experience deemed appropriate. Current unrestricted NSW driver's licence.
* Can demonstrate ability to provide leadership that facilitates the ongoing development of evidenced based clinical practice, experience in education, clinical leadership and mentoring and supervision.
* Can demonstrate ability to work within the frameworks of acute and primary health care service delivery models autonomously and as part of a team.
