We're looking for a care finder to join our purpose-driven team, supporting our clients who are vulnerable, isolated, and experiencing disadvantage, helping them to navigate the aged care system and connect with appropriate services.

About the Role
As a care finder, you will play a critical role in improving outcomes for older people who may struggle to access support due to factors such as language, cultural background, disability, homelessness, or social isolation.
Your responsibilities will include:
* Providing specialist and intensive assistance to individuals who need help interacting with My Aged Care and navigating assessments.
* Building trust and engagement through personalised and individual support.
* Collaborating with local service providers, community organisations, and stakeholders to strengthen referral pathways and provide wraparound care.
* Collecting and managing client data in accordance with care finder program requirements, including uploading to the national reporting portal.
* Actively contributing to a supportive, inclusive team culture and passionate about making a difference.
This is a part-time position (3 full days per week or 45.6 hours per fortnight), with travel required across the Great Southern region to meet people where they are in their homes, communities, or safe places of choice.

Our team are looking for someone who has a high degree of compassion and cultural understanding, someone who is empathetic and a great communicator.
Our must haves are:
* Qualifications or relevant experience in aged care, human services, or a related field.
* Lived or professional experience supporting vulnerable populations and the ability to build trusting relationships.
* Background in client-centred care.
* Knowledge of aged care services and referral systems, use of client databases and navigating reporting portals.
* A valid 'C' class driver's licence and National Police Clearance
* National Disability Clearance Check.
