The Position
The clinical care coordinator plays a crucial role in delivering person-centered clinical care to residents, adhering to established policies, procedures, and professional standards. Working closely with the care manager, the primary purpose is to focus on individual resident needs.
This involves ensuring the existence of a current and well-documented care plan that effectively addresses residents' needs. The care coordinator also contributes to the review and delivery of the care plan with the support of the multi-disciplinary care team. Day-to-day reporting for this position is directed to the care manager or general manager.
1. Duties and Responsibilities
2. To assist with the admission of new residents to the facility, ensuring they are assessed.
3. To assess the individual needs of residents and develop person-centered care plans in consultation with other healthcare professionals, ensuring that care is tailored to meet their specific needs.
4. To collaborate with other healthcare professionals to deliver comprehensive care to residents.
5. To communicate effectively with residents' families and healthcare providers, keeping them informed about their health conditions, care plans, and any significant changes or concerns.
6. Maintain accurate and up-to-date resident documentation, including care plans, behaviour charts, medication charts, observations and/or incidents.
7. To provide and assist with direct nursing care to residents, including activities of daily living (ADLs), administering and managing medication, catheterisation, conducting observations and monitoring, and wound management.
8. To actively participate in case conferences and relevant meetings.
9. To complete clinical assessments and reviews at required intervals and escalate as required, including COD status, daily wound care and fall management.
10. To undertake audits as required, including the Schedule 8 drug count.
11. To assist with pharmacy ordering as required.
12. To check alerts, overdue tasks and progress notes to ensure they are completed effectively and to standard.
13. To coordinate infection control measures are in place and ongoing risk reduction strategies are being implemented.
14. To assist with the investigation of clinical incidents and their close out as required.
15. To collate data and prepare information for reports as required.
