About UsThe Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development, and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Queensland Region.The Paediatric Services team aims to support parents and children, from early childhood through to teenage years, to enhance family functioning and promote childhood development and parental wellbeing.About the RoleAs the Paediatric Care Coordinator within the IUIH network, the role is to coordinate care for children from early childhood through to teenage years and their families. This role will entail both coordination and direct clinical care. It is a fixed-term full-time position with potential for extension, primarily based between our MATSICHS Morayfield and Caboolture Clinic locations.Working within the IUIH model of care, you will be required to:
Coordinate care for the paediatrician and other paediatric specialist services
Deliver clinical services including assessment of the infant and child
Initiate and participate in case management and review meetings with both internal and external stakeholders
Ensure children have a current completed 715 health check
Develop and maintain effective linkages with external services and agencies
Collaborate with internal services such as Allied Health services
About You
Current C Class Drivers Licence (QLD)
AHPRA Registration
Bachelor of Nursing Qualification or equivalent
Valid Disability Worker Screening Check (QLD Yellow Card) – prior to your commencement
Strong attention to details with a focus on clinical assessment and preventative health checks
Demonstrated ability to work autonomously in a highly skilled role
Desirable, but not mandatory
Demonstrated experience working with Aboriginal and Torres Strait Islander people and/or communities
Postgraduate qualification in Child Health, Public Health or related field
