We are seeking a dedicated Care Coordinator to join our team, working closely with women to support their goals, recovery, housing needs, and community integration.

This role involves delivering compassionate case management tailored to each client's journey, using trauma-informed and strengths-based approaches. You will assess risks, assist with housing, health, legal, education or wellbeing goals, and maintain accurate records.

As a Care Coordinator, you will also provide outreach support, collaborate with other services, and contribute to a positive team culture focused on safety, empowerment, and client-led care.

You will bring experience in case work or community services, a deep understanding of trauma and homelessness, and a passionate commitment to supporting women.

The ideal candidate will have:

* Relevant tertiary qualifications in social work, human services, or a related field;
">
* A minimum of 2 years' experience in case management or similar support roles;
">
* Strong knowledge of trauma-informed, strengths-based, and recovery-oriented practice;
">
* Excellent communication, time management, and crisis response skills;
">
* A belief in the inherent worth and potential of every woman;
">
">
In addition, you will be required to hold an Unrestricted Driver's Licence, undergo DHS Disability Services Employment Screening / NDIS Worker Screening, complete Child Safe Environments Training, and obtain a First Aid Certificate.
