Key Responsibilities
* Assess and understand individual client needs and preferences to create tailored care strategies.
* Coordinate and oversee client-centered care planning to ensure services are appropriate, responsive, and person-focused.
* Manage and monitor the delivery of in-home support services, ensuring full compliance to Aged Care Quality Standards.
* Build and maintain strong client relationships by providing ongoing support, communication, and guidance throughout their care journey.
* Collaborate with multidisciplinary teams to document, review, and ensure compliance with care plans and all related activities.
Required Skills and Qualifications
* Commitment to delivering high-quality care that aligns with our organization's mission and values.
* Certificate IV in Aged Care, Certificate IV in Disability, Certificate IV in Community Services, Diploma of Community Services (Case Management), or Diploma of Ageing Studies and Services.
* Experience in aged care, home care, or community care, with a passion for client-focused service delivery.
* Strong ability to develop, implement, and review personalized care plans.
* Excellent communication and relationship-building skills, with a commitment to advocacy and person-centered care.
* Knowledge of Aged Care Quality Standards and compliance requirements.
* Experience working collaboratively with healthcare professionals, carers, and external providers.
* Strong problem-solving, organizational, and coordination skills.
* Experienced in operating effectively in a multi-site environment.
* A current driver's license and safe driving record. Willingness to travel between sites and services.
* Excellent communication skills and ability to communicate with clients from diverse backgrounds.
* Strong administrative and computer skills.
* Ability to work independently and as part of a team.
