As a Social Worker in the Aged Care Assessment Team your primary responsibility will be to conduct comprehensive aged care assessments and provide referrals, support, and information to help clients navigate access to Government-funded aged care services and alternative support options.
You will play a crucial role in ensuring efficient, effective, and timely assessments for clients in both acute care facilities and community settings, promoting uniformity and equity of access to Australian Government-funded aged care services.

**A little about us.**
David Berry Hospital, located on Dharawal Country in the township of Berry, has proudly served the community since 1909, providing essential sub-acute rehabilitation and specialist palliative care. However, due to its 115-year-old infrastructure, it is transitioning its services to Shoalhaven Hospital, the main acute care facility for the region. These services will continue at David Berry Hospital until the completion of the redeveloped Shoalhaven Memorial Hospital in 2026.
Shoalhaven Memorial Hospital, situated on Yuin Country, offers a wide range of medical services, including emergency care, surgical and orthopaedic services, and has recently undergone significant redevelopment.
