At KonnectUs, our highly experienced team of Support Coordinators works collaboratively with NDIS participants to navigate the NDIS system, connect with appropriate service providers, and achieve meaningful goals.
We coordinate a wide range of services, including allied health, personal care, employment support, housing and community access, by engaging with NDIS providers, mainstream services, government departments and private health professionals.
We have developed a particular strength in working with participants who experience psychosocial disabilities, supporting them with compassion, consistency and trauma‑informed approaches.
Our team has the knowledge and experience to assist individuals with mental health challenges to build capacity, increase independence and improve their overall well‑being.
We are skilled in managing complex situations, including crisis planning, hospital discharges, homelessness and recovery‑oriented practice.
Role Description
This full‑time hybrid role for a Support Coordinator / Recovery Coach is located in Mandurah, WA, with flexibility for some work from home.
The role involves providing support coordination and mental‑health support services to individuals with disabilities, ensuring they are connected with appropriate service providers while strengthening their capacity to achieve goals.
Day‑to‑day tasks include liaising with NDIS participants and stakeholders, coordinating various services, assisting with planning and goal setting, addressing any barriers to receiving effective support, life coaching and social and emotional support.
Required Knowledge, Skills & Experience
* Tertiary qualification in a relevant field (e.g., Disability, Social Work, Mental Health)
* Strong written and computer skills for creating plans, programmes, reports, correspondence and other documents
* Experience working in disability, mental health, community services or case management (NDIS experience highly regarded)
* Strong understanding of the NDIS, psychosocial disabilities and trauma‑informed care (desirable)
* Excellent communication, organisational and interpersonal skills
* Cultural competence and a commitment to inclusive practice
* Ability to work independently and manage a diverse caseload
Valid Driver's Licence and Access to a Reliable Vehicle
* Current Working with Children Check, NDIS Worker Screening Check and National Police Clearance (or willingness to obtain)
* Strong coordination, leadership and peer support skills
* Relevant lived experience strongly considered

* Internal and external clinical supervision provided
