Clinical Nurse Consultant – Diabetes Educator (CNC CDE) | Permanent Full Time | Monday to Friday (No weekends or shift work)
Cairns Base | Regular Travel to Cape York Communities

About the role
As a Clinical Nurse Consultant – Diabetes Educator, you will:
* Deliver advanced diabetes education and clinical care across Cape York communities
* Work in genuine partnership with Aboriginal and Torres Strait Islander peoples, families, and communities
* Collaborate with Directors of Nursing and multidisciplinary teams across Primary Health Care Centres
* Lead prevention, education, and care planning initiatives
* Drive quality improvement and service development
* Contribute to reducing preventable hospital admissions through proactive, community-based care

The travel (this is a core part of the role)
* Regular travel via light aircraft and 4WD
* Staying in community 2–3 nights most weeks
* Travel allowances provided
* Balance between outreach and Cairns-based work
This is consistent, hands-on community engagement, not occasional outreach.
