Daily Responsibilities:
1. Conduct regular health assessments and monitor disease progression and treatment responses.
2. Develop care plans and coordinate with healthcare professionals.
3. Anticipate and manage changing health needs of patients.
4. Support patients and families by responding to inquiries and feedback.
5. Maintain accurate clinical documentation, including referrals, in a timely manner.
6. Administer prescribed medications and treatments.
7. Participate in or lead chronic disease management programs.
The Successful Candidate:
* Experience in the chronic disease field or postgraduate qualification in chronic disease.
* Bachelor of Nursing and AHPRA registration.
* Full working rights within Australia.
* Willingness to work in a regional setting and embrace adventure.
* Ability to pass a national police clearance.
* Highly organized, capable of working under pressure, with strong time management skills.
