We are seeking a Discharge Patient Flow Concierge Officer to join our team. This position reports to the Ward Clerk Support Manager. The role will play a key part in supporting administrative processes for discharge planning and assisting patients and their families as they transition from hospital to home or another care setting.

In this position, you will strengthen existing discharge planning processes in collaboration with the multidisciplinary team to support timely, well‐coordinated discharges. You will help ensure smooth integration with out‐of‐hospital and primary care services, enhancing the overall discharge experience for patients, carers and families. Your work will help ensure that everyone involved in the patient's care is informed, supported and aligned throughout the discharge process.
As a senior administration officer, you will also demonstrate a strong commitment to the organisation's safety management system. This includes promoting a positive health and safety culture, consulting with workers on WHS matters, maintaining up‐to‐date WHS knowledge, ensuring staff understand their safety obligations, and reporting any incidents, hazards or concerns in the NBMLHD IMS+ system within 24 hours. You will take appropriate action to eliminate or minimise risks as far as reasonably practicable.

What you will bring to the role

* Demonstrated extensive experience in customer service.
* Demonstrated high level administrative skills with ability to prioritise and multi-task
* Excellent interpersonal, communication and negotiation skills, in particular the ability to interact effectively with a multidisciplinary team, patients, and carers.
* Demonstrated ability to work autonomously, utilise discretion and maintain strict confidentiality whilst working as effective team member
* Adaptable to change with high attention to detail with ability to identify and implement change to facilitate system and process improvements
* Demonstrated ability to problem solve and determine solutions.
* Demonstrated high level IT skills and knowledge of Microsoft Office software, including Excel and proficient keyboard skills.
