Belong at Home is seeking enthusiastic, motivated and caring people to join our South West team as a Community Nurse (RN).
We have either a permanent part time or casual position available for a Community Nurse with a highly respected not-for-profit community organisation providing care and support services for Busselton and the surrounding areas.
As a Community Nurse you will:
* Undertake clinical assessments of home care clients.
* Providing excellent care and service to clients in line with their personalised care plans.
* Monitoring chronic diseases and palliative conditions.
* Wound care, continence care, palliative care, medication management and management of chronic diseases.
* A team player you will work closely with our care partners to ensure quality service delivery.
* Achieving the optimum care for clients by being responsive to their changing needs.
* Liaising with and referring to other organisations as required to provide for the clients needs.
* Assist the clinical leads with other duties as required.
Essential:
* Current AHPRA registration.
* Availability to work (M-F) - (adhoc weekends where required). Minimum 2-3 days a week.
* Aged Care experience is an advantage.
* Excellent verbal and written communication skills.
* A genuine enthusiasm, care and compassion in your commitment to working with the elderly.
* A current "C/CA" class WA Driver's License is essential.
* A National Police Clearance is required.
* Influenza Vaccination required.
