About This Role
Your new role: As An Advanced Practice Clinician, You Will Lead The Delivery Of Comprehensive, Evidence-based Geriatric Care Within a Collaborative Multidisciplinary Environment.
* Providing advanced assessment, diagnosis, treatment and care coordination for older patients with complex conditions
* Leading nurse-led models of care to improve patient outcomes and system efficiency
* Working across inpatient and outpatient settings within the Acute Care of the Elderly service
* Prescribing, interpreting diagnostics and implementing evidence-based interventions within scope of practice
* Partnering with multidisciplinary teams to deliver integrated, person-centred care
* Driving quality improvement, service innovation and clinical excellence in geriatric care
* Supporting care navigation, referrals and continuity across hospital and community settings
About You
You are a highly skilled and autonomous Nurse Practitioner with a passion for improving outcomes for older persons, bringing:
* NMBA endorsement as a Nurse Practitioner with a Master’s qualification (mandatory)
* Advanced clinical expertise in geriatrics, including frailty, cognitive impairment and complex comorbidities
* Proven ability to deliver comprehensive assessments, prescribe and manage patient care independently
* Strong leadership capability with experience guiding multidisciplinary teams and driving service improvements
* Demonstrated commitment to evidence-based practice, research and continuous quality improvement
* Excellent communication, collaboration and stakeholder engagement skills
* A patient-centred approach with a focus on innovation, safety and quality outcomes

We offer a range of services, including medical, rehabilitation, maternity, surgical and paediatric care. We provide extensive support and training in most medical specialties. As a teaching hospital, we help train future health care professionals and participate in research through strong partnerships with Queensland's leading medical schools, universities, and TAFE Queensland. Our services will continue to grow as we build bigger and better facilities to support the community. By 2027, we'll have more beds and five more birthing suites as part of the Logan Hospital expansion.
