Nurse Practitioner - Diabetes Role Overview


The North West Diabetes Centre is a multidisciplinary care team responsible for delivering comprehensive diabetes services in the Tasmanian region. Our centre provides cutting-edge care to patients with type 1 and complex type 2 diabetes, leveraging both inpatient and outpatient services.


We offer education and training programs to healthcare professionals and educators, equipping them with essential knowledge for effective diabetes management.


Our skilled team comprises an endocrinologist, nurse practitioner, unit manager, credentialled diabetes educators, nursing staff, dietitian, podiatrist, psychologist, and administrative support.


This position plays a vital role in our centre, focusing on the development, delivery, and evaluation of diabetes clinical/community nursing care that ensures optimal patient outcomes.


The successful candidate will work closely with our team to improve clinical service outcomes through research and quality improvement initiatives, participating in statewide and national activities in research, publication, and education within the specialty of diabetes.


As a leader, advisor, and mentor, this nurse practitioner will guide other senior nurses in the field of diabetes.


Key Responsibilities:



* To provide high-quality diabetes clinical/community nursing care that ensures optimal patient outcomes.

* To develop, deliver, and evaluate diabetes clinical/community nursing care plans.

* To participate in research and quality improvement initiatives to enhance clinical service outcomes.

* To lead, advise, and mentor other senior nurses in the field of diabetes.



Requirements:



* Endorsement to practice as a Nurse Practitioner in Australia.

* Advanced competence in evidence-based nursing practice in diabetes, with awareness and understanding of internal and external service needs.

Eligibility:
