ABOUT THE ROLE
We are looking for aCare Partner, Registered Nurse who is a passionate health and aged care professional looking for a current opportunity. As Care Partner you will be an integral point of contact for workers and clients to provide support for the broader community team. Join us on a permanent full-time capacity at Right at Home Southern NSW, located in Wagga Wagga.
To be successful you must hold:
- Formal tertiary qualification eg. Bachelor of Nursing or equivalent
- Current AHPRA registration as a Registered Nurse (Mandatory)
- National criminal history/police check
- Valid First Aid and CPR certificate
- Valid Working With Children's Check Card
Experience:
- Knowledge of the aged care industry/issues or Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles and the Aged Care Quality Standards – Essential
- Previous experience in Aged Care or related discipline – Desirable
- Easily adaptable to new software platforms
As aCare Partner you will be responsible for:
- Building strong and meaningful connections with clients and their family
- Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home
- Interacting and providing professional health and clinical care advice and direction to a team of home care workers
- Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction
- Ensuring all practices are consistent within policy and legislative frameworks
- Maintaining up to date client records and information
