Job Title: Telehealth Liaison
About the Role:
We are seeking a dedicated individual to join our team as a casual Telehealth Liaison. This role involves providing regular phone calls to clients, asking specific questions to gauge their health and well-being. The Telehealth Liaison will enter client responses into computer software in real-time, generating flags when a client's health may be deteriorating. This allows Care Coordinators to review calls and arrange timely interventions.
Responsibilities:
* Provide consistent phone support to clients and their carers
* Enter client responses into software for flagging potential health concerns
* Work closely with Care Coordinators to ensure timely interventions
* Engage with a diverse range of people in a compassionate and caring manner
Requirements:
* Caring and compassionate nature
