: Ongoing training and development opportunities Friendly and supportive team environment Permanent full time, Hawthorn location About the Role Under the guidance of the Team Manager and Team Consultant Psychiatrist, and in conjunction with team members the Enhanced Care Clincian will provide short to medium term case management for a small nuber of consumers who require additional specialist, targeted therapeutic interventions.
Interventions provided will be aimed at supporting psychosocial recovery, fostering individualised recovery, independence, and improved health outcomes for consumers that have been identified as having severe and complex needs.
Interventions provided could include, but not be limited to, cognitive behavioural therapy, family therapy, peer support, interventions focused on activity/vocation or additional treatment interventions to meet physical health or drug and/or alcohol needs.
Build a therapeutic alliance with identified consumers and their family/carers and actively engage them in the planning, delivery and evaluation of their care.
Assist in the provision of primary, secondary and tertiary consultation services, community development and education, liaison and linkage to other agencies that also provide support to people with mental illnesses as required.
This includes participating in and contributing to locally held portfolios.
Maintain all documentation in line with SVMH policies, including clinical documentation, outcome measures and CMI reporting.
Classifications for this position will range from RPN Grade 3 NP81 - NP74 ($100,932 - $112,091 per annum pro rata) OT/SW Grade 2 YB20 - YB23 or YC42 - YC45 ($88,030 - $101,197 per annum pro rata) Psychologist Grade 2 PK1 - PK4 ($94,478- $106,906 per annum pro rata) Your Contribution A minimum of at 2 years post qualification clinical experience within Mental Health for SW, OT and RPN.
RPNs - Nursing and Midwifery Board of Australia (NMBA).
Relevant postgraduate qualification in mental health.
Occupational Therapists - Registration with the Occupational Therapy Board of Australia.

LOCAL WORK ENVIRONMENT St. Vincent's Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara.
The adult mental health service components are: The Footbridge Community Care Unit (CCU), the 44 bed Acute Inpatient Service (AIS) located on the St. Vincent's Hospital site, the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS).
Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) Service and the Primary Intervention Care Team (PICT) includes Extended Triage and HOPE (Suicide Prevention), Continuing Care (CCT), Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services.

Experience in the provision of assessment, discipline specific intervention and case management services for people with serious mental illness and complex needs and their carers.
Demonstrated ability in working as part of the multi-disciplinary clinical team in promoting a standard of excellence in clinical practice.
This includes contributing to team clinical processes such as Clinical Review and supervision for staff and students.
A comprehensive understanding and working knowledge of the Mental Health Act **** (including amendments) and other relevant legislation.
Commitment to work collaboratively and effectively as part of the multidisciplinary team with multiple key stakeholders both internal and external of the St Vincent's Health organization (including consumers, carers, family members and community agencies).
Demonstrated ability to working collaboratively with clients and carers to promote self-determination and enable progress towards reaching personal goals.
Highly developed interpersonal and communication skills, including problem solving ability and conflict resolution skills, as well as sound oral and writing skills.
Occupational Therapy: Knowledge of and commitment to providing a comprehensive range of occupational therapy assessments and interventions for people with serious mental illness and complex needs, and their carers.
Understanding of evidenced based practice and models of occupational therapy (including the Model of Human Occupation) and their application in clinical practice.
Knowledge of sensory modulation theory and intervention.
Knowledge of the specific assessments including the MOHO Assessments and Assessment of Motor and Process Skills (AMPS).
Psychology: Knowledge of and commitment to providing a comprehensive range of psychological assessments including diagnostic, cognitive and personality assessments for people with serious mental illness and complex needs.
Experience in the development and application of evidence based therapeutic interventions for people with major mental health disorders.
Comprehensive understanding of the theories of human behaviour and psychopathology and their application in clinical practice.
