Senior Case Manager Role
We are seeking an experienced professional to play a critical role in the assessment and management of participants with complex needs on our Court Link program.
You will be responsible for providing information and advice to court users and stakeholders about all programs and supports available through our courts. You will also develop and maintain effective relationships with stakeholders, including government and non-government organisations, to initiate, negotiate and facilitate linkages and access for participants to a range of appropriate services.
In this role, you will have the opportunity to work with participants who require assistance with addressing housing, employment, drug and alcohol, health, and other social needs. You will provide interventions to address issues related to offending behaviour and make community referrals for those participants who are not recommended for case management.
You will also coordinate and negotiate the delivery of a range of services requiring the maintenance of strong linkages with the community services support sector. You will provide information to the courts regarding the assessment, intervention, and engagement of people with Court Link that may assist in bail and sentencing decisions.
Key Responsibilities:
* Assess the risk and needs of participants and develop case management plans
* Develop and maintain effective relationships with stakeholders
* Initiate, negotiate and facilitate linkages and access for participants to a range of appropriate services
* Provide interventions to address issues related to offending behaviour
* Make community referrals for participants who are not recommended for case management
* Coordinate and negotiate the delivery of a range of services
Requirements:
* Experience working with participants with complex needs
* Strong communication and interpersonal skills
* Ability to develop and maintain effective relationships with stakeholders
* Knowledge of the Courts Innovation Program and its initiatives
* Ability to work independently and as part of a team
