Job Summary:
* The Mental Health and Housing Liaison Officer (MH&HLO) will link people who require mental health services who are homeless, or at risk of homelessness, to housing services and help them access and/or sustain tenancies.
* This role will improve the connection, pathways and collaboration between Mental Health Services and Homes NSW to achieve better outcomes for consumers in accessing and/or sustaining tenancies.
Key Responsibilities:
* Utilising expertise in mental health and housing models and pathways to support complex care coordination to ensure community tenure and:
- access public and social housing
- maintain tenancies through support packages such as the Housing and Accommodation Support Initiative (HASI) and Community Living Supports (CLS) program.
* Liaising with key Mental Health teams, Housing teams, and community managed organisations (CMOs), project management, and consultancy.
* Forming part of local Housing and Mental Health 2022 (HMHA 22) committees, which will be an important touch point for Health, Housing and other partner agencies in the district.
