As aClinical Case Manager (also known as a Senior Health Coach), you'll guide older Australians through a transformative 8-week journey in ourShort-Term Restorative Care (STRC)program. This isn't about ticking boxes—it's aboutcoaching, empowering, and coordinating carethat helps people reclaim their independence and confidence.

Your Day-to-Day Might Include:
* Visiting clients at home to co-design personalised care plans that focus onreablement and wellness.
* Running engaging group sessions that promotehealthy living and self-management .
* Collaborating with GPs, specialists, and other health professionals to delivertruly holistic care.
* Using digital tools to provide virtual coaching and support.
* Building strong local networks and promoting the STRC program in your community.
What You'll Bring to the Table:
* Adegree in Nursing or Allied Health, with currentAHPRA or relevant registration .
* At least3 years' experience in clinical care, health coaching, or care coordination—especially in aged or community care.
* A passion forhealth promotion,preventative care, andclient empowerment .
* To confidently navigate digital platforms for virtual care delivery, accurate documentations and communication a high-level computer skills is essential
* A valid driver's licence and a love for getting out into the community.
