Clinical Hospital Pharmacist Role Overview
We are seeking a highly driven and team-oriented pharmacist to deliver exceptional customer service with sensitivity, compassion, and professionalism.
As a key member of our hospital team, you will be responsible for providing high-quality patient care through the preparation and delivery of medication lists, conducting thorough medication chart reviews, and coordinating patient medication requirements.
* Key Responsibilities:
* Prepare accurate and comprehensive Medication History lists for inpatients
* Conduct regular medication chart reviews and reconciliation
* Dispense and supply medications and pharmacy products in a timely and efficient manner
* Coordinate patient medication requirements for inpatient stays, day infusions, and discharges
* Develop and maintain professional relationships with key stakeholders to ensure excellent customer service to clients and patients
* Support patients, carers, hospital staff, and associated employees through education and guidance on health promotion, disease prevention, drug selection, and proper use of medicines
Requirements
* Full general registration as a pharmacist in Australia (AHPRA) or New Zealand (PCNZ)
* Previous dispensing/clinical experience in an Australian or New Zealand based hospital or community pharmacy is essential
* Previous experience in a clinical setting is highly regarded
* Strong knowledge of the Pharmaceutical Benefits Scheme (PBS) is highly regarded
* Effective time management, communication, and teamwork skills are required
* Flexibility and adaptability in working with a diverse team are essential
* Ongoing learning and commitment to professional development are valued
* Attention to detail and ability to work autonomously are critical
