What you'll be doing
As the Senior Mental Health Clinician for the Pathways to Community Living Initiative (PCLI), you'll play a pivotal role in helping long-stay adult patients with complex mental health and ageing issues transition safely and successfully back to community life.
You will:
* Deliver recovery-oriented, person-centred assessments, care planning and coordination for older adults with severe and persistent mental illness
* Provide specialist consultation, supervision, mentorship and education across inpatient and community teams
* Lead practice change and service improvement, contributing to quality, safety and innovation in care delivery
* Collaborate with consumers, carers, RACFs, NGOs and multidisciplinary teams to enable holistic recovery and community reintegration
* Support evidence-based interventions that foster empowerment, independence and wellbeing

Essential Requirements:
* Qualification: Bachelor degree or higher level
* Registration: Demonstrated full registration with AHPRA as an Occupational Therapist, Nurse or Psychologist, or eligibility to be a member of the AASW as a Social Worker (Registered Nurse requires at least 5 years full time equivalent post registration experience and approved post registration nursing qualifications relevant to the position)
Selection Criteria:
* Demonstrated skills in mental health and discipline specific assessments and interventions and experience working with older people with severe and persistent mental illness.
* Proven recent experience in a mental health leadership and/or change management role.
* Proven clinical experience within a multi-disciplinary team with the ability to provide consultancy, supervision, education and training on clinical issues.
* Demonstrated advanced written and verbal communication skills with computer competence and literacy, including the use of database systems.
* Demonstrated ability to collaborate meaningfully with consumers and carers with a commitment and understanding of recovery principles in mental health service provision.
* Demonstrated working knowledge of NSW Mental Health Act 2007.
* Current Drivers Licence valid within Australia.
