About the Position
This frontline role manages cases and delivers tailored interventions to address family needs and child protection concerns.
Key Responsibilities
* Deliver an outreach model of complex-case management programs to families in their home environment.
* Maintain a caseload of families and provide tailored evidence-informed case management interventions.
* Engage with families, obtain their consent, and collaboratively develop a case plan to address unmet needs and child protection concerns.
