Daylesford Hospital invites Expressions of Interest for the role of Aged Care On-Site Pharmacist (ACOP) to support our Residential Aged Care Facility (RACF). This position is funded by the Australian Government Department of Health and Aged Care and offers an exciting opportunity for a credentialed pharmacist to contribute to high-quality, resident-centred care.
About the Role
The ACOP will work on-site one day per week to enhance medication safety, optimise therapeutic outcomes, and strengthen continuity of medication management for our residents. Key functions of the role include:
* Improving medication use and safety within the RACF, including oversight of high-risk medications.
* Supporting day-to-day medication management, including timely review of medications and prompt resolution of medication-related issues.
* Providing accessible, expert pharmacist advice to residents, families, and staff.
Credentialing Requirements
To support the transition of existing credentialed pharmacists into ACOP roles, pharmacists currently credentialed for Medication Management Reviews (MMR) will be recognised as ACOP-credentialed on a temporary basis until 30 June 2026.
Pharmacists participating in the Measure must complete additional education to gain full ACOP credentialing via an Australian Pharmacy Council (APC) accredited training program or an APC accredited Recognition of Prior Learning (RPL) pathway by 1 July 2026.

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30th November 2019. Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
