You will play a vital role in helping residents maintain their homes and live more independently. Through early intervention, person-centred planning, and cross-sector collaboration, you'll help prevent homelessness and build pathways to recovery.
* Deliver effective tenancy sustainment solutions with a trauma-informed approach
* Support up to 25 residents through individualised support plans and collaborative engagement
* Identify potential tenancy risks and proactively work with Housing Officers to prevent tenancy breakdown
* Work alongside mental health specialists, NDIS providers, and community agencies to achieve positive outcomes
* Champion best practices, lead service improvements, and contribute to program evaluation
* Be a trusted advisor to internal teams, sector partners, and support networks
