*About The Role
We are seeking an experienced and motivated
Permanent Clinical Pharmacist*
to join our Pharmacy Department. This role is responsible for the direct provision of high-quality pharmaceutical services to patients and staff across inpatient and outpatient settings, supporting safe, effective, and evidence-based medication use.
The Pharmacy team oversees the hospital formulary and medication supply, including
controlled and highly specialised substances
.
*Key Responsibilities
Clinical Services*
* Work on rotation across:
* Acute Ward
* Subacute Ward
* Outpatient Specialty Services (Dialysis and Chemotherapy)
* Urgent Care Centre
* Provide comprehensive pharmacy services including:
* Medication reconciliation
* Resolution of medication-related issues
* Supplying medications
* Provision of medicines information
* Patient counselling
* Assisting with discharge planning
* Ensure accurate, safe, legal, and timely dispensing, checking, and distribution of medications
* Review medication orders for appropriateness, safety, dosage, interactions, and legality
* Initiate medication changes in consultation with prescribers and recommend alternative therapies where appropriate
Specialty Practice
* Opportunity to provide oncology pharmacy services (as experience or interest allows), including:
* Reviewing treatment plans
* Ordering chemotherapy
* Preparation of supportive medications
* Counselling chemotherapy patients
* Completion of antimicrobial restriction alerts in Guidance MS
* Ensure appropriate drug utilisation through implementation of hospital policy and formulary guidelines, and liaison with medical and nursing staff
Medication Safety & Quality
* Regularly monitor and assess medicine management to enhance medication safety in accordance with AdPha standards
* Assist with drug use monitoring, antimicrobial stewardship, and safety initiatives
* Participate in and initiate audit activities as required
* Design, implement, and contribute to quality improvement initiatives
* Assist with the development and review of clinical guidelines, protocols, and policies
* Take proactive steps to implement best-practice standards across all areas of scope
Education & Team Support
* Manage patient education to improve medication knowledge and adherence
* Provide medicine-related education to clinical staff to enhance medication safety and quality use of medicines
* Supervise and support technical and pharmacy staff
* Assist team members in delivering a high-quality pharmacy service
* Demonstrate proactive problem-solving to support individual patient care and service development
* Self-monitor performance against KPIs, operational targets, and strategic goals
Key Selection Criteria
*Experience & Knowledge
Grade 2 – Desirable*
* Recent hospital pharmacy experience
Grade 3 – Essential
* Minimum 5 years' hospital and clinical pharmacy experience (Exceptions may be considered for candidates with less experience who demonstrate exceptional suitability)
* Demonstrated high level of current clinical pharmacy knowledge and practice
Qualifications
*Essential*
* Current registration as a Pharmacist with AHPRA, with no restrictions on scope of practice
* Bachelor of Pharmacy (or equivalent)
* Current National Police Check and Working with Children Check
* Evidence of required vaccination status and serology
Desirable
* Relevant or working towards postgraduate pharmacy or health qualifications
* Membership of relevant professional bodies (e.g. Advanced Pharmacy Australia – AdPha)
