The Community Palliative Care Team seeks a Nurse Practitioner to deliver high-quality, patient-centered care in the client's home. This role involves comprehensive holistic assessments, symptom management, and emotional support for individuals with life-limiting illnesses.
The ideal candidate will have current AHPRA registration as a Registered Nurse and experience in palliative care or a strong willingness to learn. They must possess excellent communication skills, emotional intelligence, and the ability to work collaboratively within a team.
