The team forms part of the St Vincent’s Virtual & Home that has admitted services across Acute and Subacute streamed Units including Hospital in the Home, Rehab@home, GEM@home, Transition Care Bed Based. Non admitted services including District Nursing, Residential in Reach, Palliative Care@home, Transition Care Program, Home Based Allied Health.Our team includes: Physiotherapy, Occupational Therapy, Speech Pathology, Podiatry, Dietetics and an Allied Health Assistant.
