End-of-Life and Restorative Care Clinical Coordinator
This role involves overseeing restorative care and end-of-life care pathways within a support program.
* Closely collaborate with care partners to ensure clients are properly supported into these pathways.
* Conduct thorough assessments, identify risks, and develop tailored plans of action.
* Ensure adherence to relevant health guidelines and governance processes.
The ideal candidate will possess expertise in restorative care, palliative/end-of-life care, and related reforms. They should also have experience leading multidisciplinary teams and working effectively with families.
About You
* Must be currently registered as a Registered Nurse with the relevant authority.
* Demonstrate strong written, verbal, and interpersonal communication skills.
* Exhibit attention to detail and organizational capabilities.
* Display empathy and discretion when handling sensitive matters.
