**Job Title:** Heart Failure Clinical Social Worker
We are seeking a dedicated clinical social worker to provide essential support to patients with heart failure. The successful applicant will work closely within a multidisciplinary team, delivering psychosocial assessments and interventions, supporting patients with complex health and social needs, and engaging in supervision, training, and ongoing development.
Key Responsibilities:
* Deliver compassionate, evidence-based care to patients with heart failure
* Support patients with complex health and social needs
* Engage in supervision, training, and ongoing development
* Contribute to multidisciplinary care planning
* Maintain accurate records and departmental reporting
Requirements:
* Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW)
* Demonstrated core competency skills in psychosocial assessments
* Demonstrated understanding of issues associated with domestic violence, child protection matters, acute and chronic illness, age-related illnesses, guardianship, and end-of-life matters
* Demonstrated clinical experience or understanding of grief and loss counselling, crisis intervention, discharge planning, and working with complex family dynamics
* Demonstrated understanding of matters associated with working with people from Aboriginal and Torres Strait Islander background as well as people from culturally and linguistically diverse backgrounds
