Position Title: Community Palliative Care Nurse
Classification: ZF4 – Clinical Nurse Consultant A
Salary per hour: $59.79 + superannuation + generous salary packaging options
Employment Type: Fixed Term Contract Part-time 64 hours per fortnight until 1st September 2027
Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028
Role description
Our Community Palliative Care Program focuses on improving quality of life through a palliative approach that emphasises early identification, specialised nursing assessment, and the management of pain and other physical, psychosocial and spiritual needs.
As the Community Palliative Care Nurse, you will work in partnership with patients, families, carers and multidisciplinary teams to ensure high‐quality, evidence‐based care is delivered in the right place, at the right time.
Key Responsibilities
* Provide holistic, evidence‐based palliative nursing care in the community setting
* Undertake comprehensive palliative assessments and develop individualised care plans in partnership with patients and families
* Work collaboratively with medical practitioners and allied health professionals to support coordinated care
* Liaise closely with District Nursing and At Home nursing teams to ensure integrated service delivery
* Coordinate and participate in post‐bereavement follow‐up and community bereavement activities
* Support the Community Palliative Care Program Lead in building workforce capability through education, mentoring and quality improvement initiatives
About you
* Registered Nurse with current AHPRA registration
* Demonstrated experience or strong interest in palliative care
* Excellent assessment, communication and care‐planning skills
* Ability to work collaboratively within multidisciplinary and community‐based teams
* Commitment to person‐centred, culturally safe and compassionate care
* Experience in community nursing (desirable)
