**Social Work Services**
Social Work across Monash Health is responsible for the delivery of a broad range of Social Work services for clients across the lifespan in the inpatient and outpatient setting.
**About Hospital in the Home**
Hospital in the Home (HITH) is the provision of acute care to public hospital patients who have their treatment continued in the comfort of their own home. This is an alternative to an inpatient stay. Nursing hours are from 7am-10pm 7 days per week.
HITH provides a holistic and individualised approach to its patients. All patient care provided aims to meet the needs of the patient and their families. The case mix of patients include conditions from a medical, surgical and oncology/haematology background. We also deliver chemotherapy treatment.
**About You**
As an experienced Social Worker with Acute Hospital experience, you will be working closely with the multidisciplinary HITH team to support the operational planning and delivery of patient care in clinics and in a community setting.
With your excellent communication skills and caring manner, you will be happy driving to patients home to provide outstanding care if required. Full drivers licence is essential for this role.
