About the role
The Community Support Worker is responsible for providing personal support to people with physical and intellectual disabilities and their families through provision of quality care within the community. He/she will provide personal care and opportunities to people with disabilities and encourage them to improve and reach their full potential.
Job Responsibilities
* Ensures that quality service is being delivered to the people we support
* Supports in the individual planning process to identify goals and objectives that meet the people's individual needs and choices
* Participates in the team structure
* Ensures that the rights, interests and needs of the people we support are addressed effectively
* Understands and respects the linguistic and cultural diversity of the people we support
* Ensures that all Lifestyle Solutions Policies and Procedures are implemented and complied with
* Implements and complies with Occupational Health and Safety Policies and Procedure

Qualifications
Job Qualifications
* Relevant experience working with people with disabilities or in a related area/field, that demonstrates a positive commitment to the duties and responsibilities of a Community Support Worker as outlined above
* Willingness to work shift-work on a 7 day roster including weekends and public holidays as required
* Excellent communication and interpersonal skills
* Ability to work effectively independently and in a team
* Computer literate with competent administrative and documentation skills
* Current Driver's License
* Current First Aid Certificate or willingness to obtain one
* Tertiary qualifications in Social Sciences, Community Services and/or experience in service delivery in the disability field is an advantage
* Knowledge and skills in various methods of service delivery including case management, casework, outreach, counselling, etc. is a plus
* Must be willing to work in Tarneit.
